AHS WAIVER & RELEASE

The AHS Single Event Waiver and Release Form is confidential and not accessible by the general membership. The AHS Treasurer will retain possession of the document and will keep the personal information on it confidential unless required to surrender it to a lawful authority and only if directed to do so by legal due process.

MINIMUM AGE REQUIREMENT AND PROOF OF IDENTIFICATION:

I hereby affirm that I am 21 years of age or older. I affirm that I am registering under my legal name. I understand that I will be required to provide valid government-issued photo ID as proof of identification and age. I agree that my registration may be canceled without refund or other consideration if I fail to register under my legal name or have attempted to purchase someone else's registration.

INDEMNIFICATION AND HOLD HARMLESS:

I agree to hold harmless and indemnify AHS, its officers, operating committee members and chairpersons, agents, members, volunteers and host establishments including, but not limited to, their Management, Owners, Operators or their officers, boards of directors, agents and/or assigns, as well as employees and staff of said businesses or facilities, from any and all liability, direct, indirect, subrogated or not, vicarious or otherwise, which may accrue as a result of any accident, injury, cause of action or damage to my person or property, or any other damage of any kind as a consequence of my attendance at this Event.

UNDERSTANDING OF SUBJECT MATTER:

I certify that I am attending this Event solely for my personal enjoyment and understand that, in so doing, I will be exposed to BDSM (Bondage/Domination/Sadism/Masochism), D&S (Dominance & Submission), S&M (Sadism & Masochism) and/or fetish activities that some people might find offensive. I further certify that I do not find such activities offensive and that I consent to being exposed to such activities while attending this AHS Event.

ATTENDING EVENT FOR PERSONAL ENJOYMENT ONLY:

I hereby certify that I will attend this Event for the sole purpose of my own personal enjoyment. Additionally, I certify that I am not an employee of any law enforcement agency or that, if I am an employee of any law enforcement agency, I am attending this Event solely for my personal enjoyment and not as part of any investigation. I further certify that I am not a member of the media/press here for any reporting or journalistic purposes or that, if I am, I am attending this event exclusively for my own personal enjoyment and not as part of any investigation or gathering of information and that I will not use any such information in any manner without the express written permission of AHS. I understand that this is a legally-binding, contractual agreement and that breach of this agreement constitutes willful misrepresentation and that, unless given express permission to cover an AHS Event at a specific place and time, that violation of these terms may result in civil or criminal action against me and any media employers who directly or indirectly use the reported information for any purpose whatsoever. Moreover I, and all of my invited guests, understand that "professional scene" people may not attend an Event as a paid escort of an attendee, nor under any agreement to provide any BDSM services or activities for compensation, nor shall they distribute any promotional or advertising materials, nor solicit professional services. I understand that prostitution, solicitation or negotiation for any sexual or BDSM activities for any consideration or any agreement to exchange sexual activities or BDSM play for cash or for anything of value is strictly prohibited and will be grounds for removal from the Event without refund or other consideration.

RULES OF ETIQUETTE AND SERGEANT AT ARMS:

I agree to comply with any instructions given to me by the AHS Sergeant at Arms (SAA) or designated representative, regardless of whether there is a violation of the AHS Rules of Etiquette, which I have agreed to under separate cover. I further agree to inform my guest(s) of this requirement. I understand that failure to follow these rules or the instructions of the SAA may result in expulsion from the Event. Should I be asked to leave, I further understand and agree that no refund or compensation will be granted or offered for any fees I may have paid.

RECORDING EQUIPMENT:

I understand that recording equipment of any kind is not allowed at any AHS Event. Recording equipment of any kind, discovered at an Event, will be temporarily confiscated and returned to the owner upon closure of the Event. Any recording device discovered to contain media recorded during an AHS event will be permanently destroyed. This includes, but is not limited to, all handheld devices (i.e. PDAs, cellphones, etc.). I agree to turn over any such device and understand it will not be returned regardless of any other contents on, in or of such media.

CONFIDENTIALITY:

I agree that everything that I observe during the Event will be kept in strict confidence and recognize this as a contractual obligation. I will do nothing to jeopardize the privacy and identities of any other attendees, members or the host establishment. I agree that, if I create any descriptions or accounts of any activities at an Event in any way or manner, I will transfer any copyrights of any such materials to AHS and waive any rights I may otherwise have to either request disclosure or inspection of any financial records or official documents of AHS, or to bring any action administratively or at law, in equity or otherwise, against AHS, its officers, operating committee, committee chairpersons, agents, members or reciprocal group members, guests or volunteers, notwithstanding any rights I might otherwise have under common law and under statutes including, but not limited to, Chapter 33 RSMo.

ACKNOWLEDGMENT:

By signing an AHS Single Event Registration Form, I acknowledge that (i) I have read all of the above rules; (ii) I fully understand all of the above rules and agree to abide by them; (iii) I assume all risks incident to my attendance at any Event; (iv) I hold harmless from any and all liability AHS, its officers, operating committee members, committee chairpersons, agents, members or reciprocal group members, guests, volunteers, and the host establishment including, but not limited to, businesses or facilities that are venues for Events, and any and all of its Management, Owners, or Operators of the host establishments, businesses or facilities, or its officers, boards of directors, agents and/or assigns, as well as employees and staff of said businesses or facilities from any and all liability, direct, indirect, subrogated or not, vicarious or otherwise, which may accrue as a result of any accident, injury, cause of action, or damage to my person or property, or any other damage of any kind as a consequence of my attendance at any Event; (v) I understand I will be required to show government issued photo ID when attending or registering for any Event; and (vi) I have read and agree to abide by the AHS Rules of Etiquette found elsewhere on this site.
